MEDICATION
Prescription medication requiring administration during school hours will be dispensed by school personnel if the medicine is provided in the original bottle with specific instructions, including name of child and physician, medication name, amount and time to be taken. A signed parent note must accompany the medication with instructions for times and dosage to be given at school. Long-term medication (any medication to be administered longer then two weeks) requires physician and parental written consent to administer. Over-the-counter medication (Aspirin, Tylenol, cough and cold preparations) and Non-prescription medicine can not be administered at school unless it is accompanied by written instructions for it's use from a physician, and signed parent request. Students may not carry medications.
Parents should feel free to come to school to administer non-prescription medicine to your child as needed.
General, medicine sent to school should be limited to that which is required to be given during school hours. Medicine prescribed for three times a day should be given at home before school, after school, and at bedtime, unless otherwise ordered by the doctor (i.e. with meals).
ILLNESS AND INJURY
First aid is provided at school by certified personnel for injuries or illness occurring at school only. Injuries that occur at home should be dealt with at home. Serious injuries that may require further medical care will be referred to the parents.
A child having a temperature of 100.4 degrees or more will not be allowed to stay in school. If your child is ill in the morning, please do not send him/her. If his/her temperature is 100.4 degrees or more, medication to control fever is not available at school. The child should remain at home until the temperature has been below 100 degrees without medication for 24 hours.
THE STATE OF TEXAS IMMUNIZATION LAWS: (Requirements for attending Texas Schools)
(A) Polio - At least three doses of oral polio vaccine are required, with at least one dose received after the fourth birthday. (B) Tetanus/Diphtheria - At least three doses of DPT and or/Td vaccines are required with at least one dose received after the fourth birthday. Booster is required every ten years. (C) Measles - All students are required to have this vaccine on or after the first birthday with a second vaccine for students whose twelfth birthday occurs on or after September 1, 1990. (D) Rubella - One dose is required. (E) Mumps - One dose is required. (F) Tb Skin Test with results - One is required when entering school, when new to the CISD and with Td booster. (G) Hepatitis B - Three doses of hepatitis B vaccine are required for the following grades for the following school years: (H) Varicella - One dose of varicella vaccine received on or after the first birthday is required. (Two doses vaccine given at 13 years or older)
EMERGENCIES
It is the parent's responsibility to provide the school with phone number changes and emergency contacts as they occur. It is imperative that we have numbers where we can reach a parent or someone who can act in his/her behalf should an emergency arise. If a child's illness or injury has the potential for being life threatening, we will call the appropriate EMS unit for treatment or transfer to a medical facility. The parent will be responsible for any and all charges. Every effort will be made to reach the parent.
PERSONAL CARE PRODUCTS
Personal care products are available to girls in the vending machine in the restroom in the nurses' office. The machine takes quarters only.
